Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

• Medicals for pre-employment, sports and driving requirements (HGV etc.)
• Insurance claim forms
• Private prescriptions
• To Whom It May Concern letters

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Where possible we are happy to accommodate requests for the following services.  However, as these requests are outside of normal NHS services, they will attract a fee.Please allow as much time as possible when requesting these services as they are not a priority and therefore we complete them as/when time permits.  As a general rule please allow at least 5 working days for letters/forms to be completed.

Private Prescription issue - £25

Holiday cancellation/BUPA/PPP forms - £50

Signature to witness (Not passport) - £35         

Firearms licence - £110

HGV/PSV/Taxi medical - From £140

Power of Attorney forms - Single forms - £50
