Register as a new patient

Please use the postcode checker below if you are unsure whether you reside within our practice area. For clinical safety reasons, we are unable to register patients who live outside the practice boundary. If you are on regular medication or have a history of serious illness, our receptionist will arrange an appointment for you with a doctor or nurse to record the necessary details about your medical history.

At Misbourne Practice, all our patients are allocated a named GP. This means you have a doctor who has overall responsibility for your care and support. However, you are welcome to see any GP within the practice at any time if you prefer. You will be informed of your named GP once your registration is complete.

We are always pleased to welcome new patients, provided you live within our practice area. Please visit our reception where our staff will be pleased to help you with the necessary paperwork. It would be most helpful if you could bring your NHS medical card. Each surgery has both male and female doctors, and you are free to consult with any doctor you wish.

Would you rather register with a paper form? Simply request a copy by sending an email to bobicb-bucks.registration.misbourne@nhs.net. Our team will provide you with the necessary form and instructions to complete your registration.

Change of Circumstances

Please help us to keep your personal details up to date by notifying us immediately of any changes e.g. change of name, address, telephone number etc. If you move outside of our practice area, you should register with a new doctor as soon as possible so that your medical records can be sent on.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one and are temporarily residing in the area you can receive emergency treatment from the practice. This is known as a temporary registration, and only lasts for a maximum of three months; after this time you will need to register permanently if you have an address in our catchment area or seek treatment from you registered GP surgery.

Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered. 

Named GP

All patients at the Misbourne and St Giles surgeries have a named, accountable GP who is responsible for coordinating their care.  Your named Doctor is allocated to you by the Practice.

Irrespective of who your named GP is, you can still book appointments with a Doctor of your choosing.  Whilst you are entitled to consult any of the GPs in the Practice, we would encourage you to remain under the care of your named GP for continuity.

If you would like to know who your named accountable GP is, please ask a member of the Reception team.

Disabled Patient Facilities

There are adequate parking, access and toilet facilities for disabled people. The staff will be pleased to help, if needed.
